In order to qualify for the Medicaid Assistance program, an individual must meet both medical and financial criteria which are set forth in State and Federal law and regulations. Persons qualifying for Medicaid must have incomes and resources below specific levels determined by each state regulating the program. Anyone has the right to apply for Medicaid. To apply for the Medicaid program you must contact the Department of Health and Human Services in the county in which you live. A caseworker will give you an application and talk with you about your needs for medical assistance. Following are a list of items you should have available for review at the time of application:

  • Social Security card
  • Birth certificate and any other type of identification, such as a driver's license or Medicare card
  • Proof of all income
  • If you receive Social Security, your Social Security award letter
  • Information about cash on hand, money in checking and savings accounts, savings bonds, current value of stocks, life insurance and health insurance
  • Information about property you own or are buying
  • Information regarding any monies or properties you have recently transferred
  • Information about medical treatment and medicines you need regularly
  • Medical bills you owe

Please remember that Medicare is the federally funded program a person automatically becomes eligible for at the age of 65 and provides very limited coverage in a skilled nursing facility. Medicaid is a state funded program, with each state determining eligibility through specific criteria, which may vary from state to state. Specific coverage under the Medicaid program will be provided to eligible recipients prior to time of admission.
